A group of Canadian health professionals has taken strong exception to Canada’s federal health agency’s growing advocacy of a “plant-based diet.” The Canadian Clinicians for Therapeutic Nutrition say there is no scientific support for such a diet, and warns that plant-based eating omits such important nutrients as vitamin B12, absorbable iron, zinc, long chain omega-3 and polyunsaturated fats. The proposed focus on a plant-based approach lacks sound scientific support. Last week, a study published in JAMA looked at the costs of unnecessary blood sugar testing in patients with type 2 diabetes who are not at risk of hypoglycemia (dangerously low blood sugar levels). The researchers looked at two years of claims data and found that one in seven patients at low risk of hypoglycemia made three or more claims for glucose monitor test strips, at an average insurance cost of $325 per person per year. NBC News: Study explains how red meat raises heart disease risk For starters, this was a well run and controlled study. Researchers randomly assigned 133 subjects to one of three isocaloric diets with the only difference being the presence of red meat, white meat, or vegetarian protein. Similar to the study by Dr. Ludwig that we referenced earlier, a strength of this study was that the study team supplied all meals for the subjects. Therefore, there was no guessing about what the subjects ate or if they complied with the recommendations. That makes this a strong nutritional study. Subjects stayed on each diet for four weeks and then had a washout period before transitioning to the next diet. I had the pleasure of admitting a gentleman in his 70’s who was transferred from a small, outside hospital for unstable angina (chest pain from heart disease) and required further workup. Of note, his glucose was elevated at 290 mg/dL (16.1 mmol/L) on presentation to the emergency department, with no prior diagnosis of diabetes at the time of admission.  When I started discussing his hyperglycemia, he recounted how he had been indulging on fine cuisine while vacationing in Italy the previous month and had since been settling back down to his usual routine. On review of his old records, it was evident that he should have been classified as “pre-diabetes” for at least the past six years. Also, for at least the last eight years, his triglyceride/HDL ratio – considered the most powerful predictor of developing coronary artery disease in one study – had been over five, indicating that he was at a markedly increased risk. There are certain things that doctors are great at. How to prescribe medications? Yes. How to do surgery? Yes. Nutrition and weight loss? No, definitely not. You might be a little stunned to hear that admission, coming from a highly trained medical specialist like myself. But, it all comes down to a physician’s training and what they see as their circle of competence. Medical training extends over more than a decade, and there is barely any attention paid to nutrition or the equally thorny question of how to lose weight. Medical training begins in medical school, where standard curricula include a mandated number of hours for nutrition which varies depending upon where you did your training.
