This guideline for treating hypertension was put out by the American College of Cardiology and the American Heart Association, and effectively lowered the definition of hypertension from 140/90 down to 130/80. The organizations also recommended drug treatment for all individuals with blood pressure greater than 140/90, regardless of underlying risk. It makes sense, and is backed by science, that fitter people are generally healthier and therefore should live longer. But what’s different about this study is that there was no maximal benefit. JAMA: Association of cardiorespiratory fitness with long-term mortality among adults undergoing exercise treadmill testing Prior studies such as The Copenhagen City Heart Study and a 2015 JAMA study suggested that there was a lower threshold of only 20-minutes per week to see a benefit, but there was also an upper threshold at around 450 minutes per week of exercise where we no longer saw a health benefit. A new analysis of earlier observational studies, published last week in PLOS Medicine, suggests that those who eat full-fat dairy products also experience better health. Last summer, we reported on a study that showed a link between eating more dietary dairy fat and lower rates of stroke. This week’s analysis reveals an association between eating more dairy fat and lower rates of type 2 diabetes. Newsweek: Can type 2 diabetes be prevented by cheese? This was a large analysis, with over 63,000 participants. On average, the authors note 29% lower risk of type 2 diabetes for those with the highest level of dairy fat consumption versus those with the lowest. Both this review, and the one showing lower rates of stroke, relied upon an objective measurement of dairy fat consumption: biomarkers in the blood. A case report in 2000 described a 24-year-old woman presented to her physicians with some unusual symptoms. During exercise, she experienced a full grand mal seizure, without any previous history of epilepsy. In the prior six months, she had become very tired and noted episodes of trembling, blurred vision and confusion. She could control these symptoms by eating something. Upon investigation, she was found to suffer from a rare insulin secreting tumor of the pancreas known as an insulinoma. This tumor was massively overproducing insulin inappropriately, causing her blood sugar to fall very low, going down as far as 1.6 mmol/L after an overnight fast. She had way too much insulin in her system. Upon further questioning, she also noted acne, hirsutism and her periods had become very irregular, varying from 40 to 44 days over the previous year. Lipoprotein (a) — or its abbreviation, Lp(a), pronounced “el-pee-little-a” — is emerging from obscurity. And for good reason. Among them, Bob Harper, the TV famous trainer from The Biggest Loser, has helped bring Lp(a) to national attention after his heart attack in 2017. Put simply, Lp(a) is a version of LDL that is more dangerous. It is more pro-inflammatory and more pro-thrombotic. Studies show that elevated Lp(a) levels correlate with increased cardiac risk. So, it makes sense that we would want to lower patients’ elevated Lp(a) levels to reduce cardiac risk, right? So far, that hasn’t been very successful. Unfortunately, Lp(a) does not respond well to lifestyle changes. Its level is almost entirely genetically determined and cannot be easily manipulated with environmental exposures (nutrition and exercise) the way that LDL can.
